Through the Chronic Disease Management (CDM) program, formerly known as the EPC Plan (Enhanced Primary Care), eligible patients can receive Medicare-subsidised physiotherapy sessions with a valid GP referral.

 

What Is Medicare Physiotherapy?

Medicare physiotherapy refers to physiotherapy services that are subsidised under the CDM program by the Australian Government. If your doctor determines that you have a chronic medical condition that requires multidisciplinary care, you may be referred for up to five Medicare-funded physiotherapy sessions per calendar year.

Accessing Medicare Physiotherapy

To access Medicare physio under this program, follow these simple steps:

  1. Visit Your GP
    Your GP must assess your eligibility and prepare a Chronic Disease Management Plan. This may also be called a GP Management Plan and Team Care Arrangement (TCA).
  2. Obtain a Referral
    Your GP will provide a referral form specifically for physiotherapy. This referral is required to access Medicare rebates.
  3. Book an Appointment with Us
    Once you have your referral, contact our clinic. We'll book your initial appointment and work with you to develop a treatment plan tailored to your needs.
  4. Claim Your Rebate
    After each session, we will process your claim through Medicare. You will receive a rebate of approximately $60 per session (based on current Medicare rates).

FAQs – Medicare Physiotherapy

Do I need a referral for Medicare physiotherapy?
Yes. You must have a valid referral from your GP under the Chronic Disease Management Plan to claim Medicare rebates for physiotherapy.
How much is the medicare rebate?
As of 2025, Medicare provides a rebate of $60.35 per session. You may have a small gap fee to pay.
What conditions are eligible?
Any chronic condition that has lasted or is likely to last 6 months or longer may be eligible. This includes arthritis, diabetes, chronic pain, heart disease, and more.
Can I use all five sessions for physio?
Yes. Your GP can allocate all five sessions to physiotherapy if it is clinically justified.
Is there a waiting period to access Medicare physio?
No. Once your GP referral is completed, you can book in immediately. We aim to offer timely appointments for all referred patients.
Can I claim Medicare physio and private health at the same time?
No. You can only use one funding source per session — either a Medicare rebate or a private health fund rebate.
Do the sessions expire?
Yes. Sessions must be used within 12 months of the referral date or by the end of the calendar year, whichever comes first.
